This plan will work for you if you need help getting back on track with your existing program.
During our initial 60 minute appointment, we will conduct a complete nutrition exam, analyze your 4 day food log, customize a meal plan and personalize a lifestyle program.
During our bi-weekly follow up appointments, we will evaluate progress, make adjustments to your program and new set goals.
As a crucial component to your program, I strive to develop and continue constant communication regarding your progress. You can expect weekly emails to monitor your plan on weeks that you will not be seen in the office.
Package includes: 1- 60 minute initial appointment, 2- 30 minute follow up appointments and unlimited text/email support!
Consultation must be purchase/conducted prior to purchasing any 6 week, 3 month or 6 month package.
After purchasing your "Fresh Start Package", you will be contacted within 24 hours to schedule your appointment.
All appointments can be done in-office (534 Larkfield Road, E. Northport, NY 11731) or virtually.
Fresh Start: 6 Week Program
I acknowledge that appointment times are individually reserved. Any and all cancellations and/or rescheduling, regardless of reasoning, must be made at least twenty-four (24) hours in advance of appointment time. Cancellations and/or rescheduling are to be made by calling 631-606-4624or emailing email@example.com. I understand that if I do not adhere to the above cancellation policy, I will be charged for the corresponding session. If I have already purchased a package deal, one (1) session will be deducted at the sole discretion of the service provider. I understand that the "Fresh Start 6 Week Plan" is to be started within one (1) month of date of purchase and will be completed within eight (8) weeks upon start date. It is my responsibility to attend my nutrition counseling appointments at the time in which each are scheduled. I acknowledge that any delays, in which I, the client, am held responsible, will not be justified cause to extend provided services. All sessions will be timed accordingly, and I do not expect my nutrition counselor to run overtime. All additional time must be approved by the service provider and I may be subject to additional service fees. I understand that "Nutrition Sliced" packages are non-refundable and non-assignable.
